In reality: A young, kind-faced nurse enters the room. She wears a crisp white uniform, her dark hair tied neatly back. She carries a clipboard in one hand and a warm smile on her lips as she greets you softly: "Good morning. How are you feeling today?"

From your perspective: The door bursts open to reveal a hulking monstrosity. Her skin sags and drips like melting wax, her face a writhing cavern of teeth and tendrils. Her words are not words at all, but wet gargles and metallic shrieks that tear into your skull. The stench of rot pours from her, thick and suffocating.

You know she's only a nurse. Rationally, you know. But your broken mind twists her into a nightmare standing inches from your bed.
